Vital Life Planner

In response to the needs of older adults related to health, daily organization, and emotional well-being, the innovative tool Vital Life Planner was created – combining the functions of a calendar, journal, workbook, and guide.The planner was developed within the international project Crea(C)tive Ageing, implemented by organizations from Poland, Italy, and Bulgaria: WIELERZECZY (coordinator), Associazione Culturale Eufemia APS, and Foundation "Situational Center Open Doors."

Who is the planner for, and why is it unique?

The Vital Life Planner was designed especially for older adults, as a response to the lack of comprehensive tools tailored to support daily life, memory, and overall well-being.What makes it truly special is that older people themselves were actively involved in its development – their input directly shaped both the structure and the content.

Unlike typical calendars or organizers, the Vital Life Planner offers holistic support – combining practical planning with healthy habits and mental and emotional activation.

What’s inside the Vital Life Planner?

This easy-to-use, senior-friendly planner includes over 100 pages, with a clear layout and space for writing. It features:

  • Hydration tracker – to help you remember to drink enough water

  • Medication reminders – to support regular intake

  • Creative and artistic activities – to stimulate memory, imagination, and fine motor skills

  • Relaxation tasks – to ease stress and reduce loneliness

  • Important phone numbers section – including space for local support and emergency contacts

  • Customizable modules – for notes, appointments, to-do lists, and more

Its open-date format allows you to start using it at any time of the year.

The Vital Life Planner is completely free to download and print.It is available in four language versions: Polish, English, Italian, and Bulgarian.

Planer jest niezwykle uniwersalny – sprawdzi się u seniorów, którzy chcą utrzymać aktywność i sprawność, ale także u dorosłych szukających sposobu na lepszą organizację dnia, większą uważność czy motywację do zdrowego stylu życia. Może być użyteczny w pracy edukatorów dorosłych, terapeutów czy trenerów – jako materiał wspierający rozwój uczestników zajęć. Dla mnie największą jego zaletą jest to, że nie przytłacza  – ani graficznie ani informacyjnie.
