Il “patient engagement” si traduce nelle abilità comportamentali acquisite/acquisibili dai pazienti per mettere in atto una buona autogestione della cura e dello stile di vita. E’ un termine-ombrello (comprensivo di altri concetti tradizionali quali l’aderenza del paziente, la compliance, l’empowerment e la partecipazione nelle decisioni condivise) connesso a quei fattori di natura psicosociale, relazionale ed organizzativa che danno forma alla capacità dei pazienti di diventare maggiormente proattivi, consapevoli e partecipativi lungo il loro “percorso” di cura (Gruman J., 2010). E’ un “paziente” più consapevole della propria diagnosi, capace di attivarsi in modo corretto ai primi segni e sintomi della malattia, di mettersi in contatto con il medico tempestivamente e di fruire dei servizi sanitari offerti dal sistema in modo più soddisfacente.

Secondo una ricerca del Piaac (Programme for International Assessment of Adult Competencies), un programma dell'Ocse che valuta le competenze della popolazione, il 28% dei cittadini italiani hanno low skills' cioè basse competenze. Ciò vuol dire che riescono a comprendere solo brevi testi su argomenti familiari.
